Latest Theme updates incorporating navigation structure change.

Moved the comment template tags for prevalence to separate lines so the markdown parser doesn’t choke on them. Paragraph tags should render properly now.

Move background colour on HTML element to sites template styles as it is site-specific

Adding page template (app).

Updated the header theme to use Set. Navigation is being dynamically generated with correct aria role being set for the active navigation item. Closes #25.

Lists of links are no longer indented. Closes #15.

Removed the top padding on pages for the app. Related to better/app#57.

Style overrides for app to remove card effect from main element. We’re now handling that natively. Part of better/app#57.
